Sign Up Instructions

1. Download the application form. Please note the i-Tel service can only be used from Japanese regular or mobile phones.

Acrobat Reader is required to open the PDF application form.
Japanese Acrobat Reader is required to open Application Form for customers in Japan.

2. Fill out the form and sign.

3. Fax or mail the form to i-Tel Customer Service.

*For Japan Automatic Bank Transfer, please send the application form in the mail. Faxes are not acceptable.

4. You will receive the service instruction in the mail as soon as the application process is completed.
